What goes on a fax cover sheet

A cover sheet is a routing page. It goes through the machine first. It tells whoever picks up the stack who the pages are for. Who sent them. How many there should be. Nothing else belongs on it.

FieldWhy it's there
Recipient nameThe fax lands on a shared machine. A name gets it to the right desk.
Recipient fax numberConfirms the pages reached the number you dialed. Not a neighboring extension.
Department or companyLarge offices route internally before anyone reads the pages.
Sender nameTells the recipient who to reply to.
Callback numberIf a page comes through unreadable this is how someone tells you.
DateThe machine stamps a time on the page. It's often wrong.
Total pages including the coverLets the recipient check the fax arrived whole before filing it.
SubjectOne line. Enough to sort the fax without reading it.
MessageOnly what the pages don't say.

Confidentiality statements

A confidentiality notice sits at the foot of the page. It tells an unintended recipient what to do. Picking a type above puts the matching wording on the sheet. Copy them into a template you keep if that's easier than coming back here.

General

This facsimile contains information intended solely for the named recipient and may be confidential. If you have received it in error, please notify the sender and destroy all copies.

Health information

This facsimile may contain protected health information covered by the Health Insurance Portability and Accountability Act. It is intended solely for the named recipient. If you have received it in error, notify the sender immediately and destroy all copies. Any review, disclosure, or distribution is prohibited.

Legal

This facsimile is sent by or on behalf of counsel and may contain attorney-client privileged and work product material. It is intended solely for the named recipient. If you have received it in error, notify the sender immediately and destroy all copies without reading or distributing it.

Wording on a page isn't the same thing as compliance. A notice tells a stranger what to do with pages they weren't meant to see. It doesn't make a transmission compliant with any regulation. Nothing here is legal advice. A rule may apply to what you're sending. Check the wording with someone who knows it.

Layouts

Modern

Ruled lines. Sans-serif. The fields spaced out. The same layout the Cheapfax app puts in front of a fax.

Traditional

A boxed memorandum in a serif face. Closer to what a law office or a hospital records desk expects.

Minimal

Every rule removed. Labels above values. Nothing else on the page.

Questions

Do I need a cover sheet to send a fax?+
Not always. It helps. A fax arrives at a shared machine or a shared inbox. The cover sheet tells whoever picks it up who the pages are for. It also gives the page count. Offices that handle records or claims or filings usually expect one.
Does the cover sheet count as a page?+
Yes. It transmits like any other page. The page count on the sheet should include the sheet itself. That's the number the recipient uses to check the fax arrived whole.
What do I put in the page count?+
The total going through the machine. Cover included. Three pages of a contract plus this sheet is four.
Is it free?+
Yes. No account. No email. No watermark. No limit on how many you make.
Does anything I type get uploaded?+
No. The preview is built in the browser. So is the PDF. Nothing you type is sent anywhere. There's no server to send it to.
Can I send the fax from this page?+
No. This page makes the cover sheet. Sending is what the Cheapfax iPhone app does at 50¢ per page with no subscription. It builds the cover sheet for you as part of the send.

The app makes this page for you.

Send a fax from an iPhone at 50¢ per page. Fill in the same fields. Pick a layout. The cover sheet goes out in front of the document. No subscription. US & Canada.
